How do I contact my case manager?

If you know who your case manager is, there are several ways to contact them:  

  1. Download this list of Case Manager Contact Information and use the phone number or email listed to contact your case manager. 
  2. Call the RMHS main line (303-636-5600) and use the phone directory to find and contact your case manager. 
  3. Email CMAQuestions@rmhumanservices.org and we will connect you with your case manager.
