Changes are being made to the Home and Community-Based Services (HCBS) case management system in Colorado, and RMHS is committed to supporting you and your loved ones through the process. Visit this page often for updates, information, and easy ways to learn more.

What is Case Management Redesign?

Case Management Redesign is the state’s plan to make it easier for people in Colorado to get the help they need and make it simpler to find the right services.

Starting on November 1, RMHS will be the one agency in charge of providing case management for people in Denver and Adams Counties, and Developmental Pathways will be in charge of serving people in Douglas, Arapahoe, and Elbert Counties.

I receive HCBS case management services from RMHS. What does this change mean for me?
 

If you live in Douglas, Arapahoe, or Elbert Counties, your case management services will automatically switch to Developmental Pathways on November 1st, and they will assign a new case manager to help you. You don't need to do anything unless you want to stay with RMHS.

If you want to stay with RMHS for your case management, you need to let us know. You can do this by contacting your case manager or clicking this button and filling out the form:

I have questions about my options and my services through RMHS. Who should I contact?

RMHS will help you every step of the way. We will provide you with information and resources. If you have questions or want to talk about your options, you can:

1. Contact your case manager

2. Email CMRDquestions@rmhumanservices.org

3. Call 844-790-7647 to talk to an RMHS representative

What will happen next?

RMHS is working closely with Developmental Pathways, and we want this change to be very easy for you. By November 1, 2023, you will get information from your assigned case management agency. This information will tell you more about what's going to happen.

For now, here’s what you need to know:

 

  1. Changes in Case Management Agency: Starting on November 1, 2023, RMHS will be the case management agency for people living in Denver and Adams Counties, and Developmental Pathways will be the case management agency for people living in Douglas, Arapahoe, and Elbert Counties.
     

  2. Your Case Management Services Will Transfer: Your case management services will automatically switch to Developmental Pathways on November 1, and they will assign a new case manager to help you. You don't need to do anything unless you want to stay with RMHS.
     

  3. You Can Stay with RMHS: If you want to stay with RMHS for your case management, you need to let us know. You can do that by filling out this form.

    844-790-7647 to speak with someone. We are here and happy to help you.
     

  4. Your Service Plan Won’t Change: RMHS will work hard to make sure  the services in your Service Plan stay the same, whether you transfer to Developmental Pathways or stay with RMHS.
