RMHS Awarded Home & Community-Based Services Case Management Contract for Adams & Denver Counties

The Colorado Department of Health Care Policy & Financing awarded the contract, effective Nov. 1, as part of Case Management Redesign Process

DENVER – Sept. 14 – Rocky Mountain Human Services (RMHS), a nonprofit organization with more than 30 years of experience providing person-centered case management services in Colorado, has been awarded the contract to serve as the Case Management Agency (CMA) for Adams and Denver Counties by the Colorado Department of Health Care Policy and Financing (HCPF). Under this contract, set to take effect on November 1, 2023, RMHS will continue to provide streamlined case management services to individuals with disabilities or other complex needs that require long-term, home and community-based services and support.

The new contract is part of HCPF’s redesigned case management system intended to make it easier for people needing services to navigate the system and get the care they need. It is designed to make sure that people have one place to go for questions and support, and to improve the quality of care they receive.

RMHS has a long history of service to individuals and families in Colorado, and its commitment to delivering exceptional case management for people needing long-term care and services is steadfast.
