Starting July 1, 2025, Community First Choice (CFC) will be a new Medicaid benefit in Colorado that helps people with everyday tasks so they can live safely and independently at home, rather than in a nursing home or other facility.
CFC will be available to eligible Health First Colorado (Medicaid) members and offers personal care services without a waitlist. It’s meant to support people in staying in their homes and communities, with independence and choice.

CFC is a Medicaid benefit that covers essential in-home support services to assist with daily living. Some of these support services include:
- Bathing, dressing, and personal hygiene
- Help with meals and eating
- Light housekeeping and laundry
- Mobility assistance
- Medication reminders
You can choose who provides your care, whether it be an agency or someone you trust.
To qualify for CFC, you must:
- Be enrolled in Health First Colorado (Colorado’s Medicaid program).
- Need assistance with daily activities.
- Be at risk of needing care in a facility like a nursing home or hospital.

- If You’re Not on a Medicaid Waiver: Contact your local Case Management Agency (CMA) to see if you qualify and to start the application process.

- If You’re Already on a Medicaid Waiver: You don’t need to do anything right now. Your case manager will discuss CFC services with you during your next annual assessment.
CFC provides basic personal care services with no waitlist for anyone who qualifies. HCBS waivers offer additional supports (like respite care or home modifications) for specific populations and may have waitlists.
If you’re already receiving waiver services, you may still qualify for CFC. Your case manager can help determine what’s right for you at your next annual assessment.
