INCLUDE is a Rocky Mountain Human Services program designed to support individuals with intellectual and developmental disabilities (I/DD) who are experiencing homelessness or housing instability.
The program combines housing navigation, stabilization support, benefits coordination, and long-term service connection to help individuals access stable housing and maintain community living.
INCLUDE provides person-centered support tailored to each individual’s unique needs and goals.

Not necessarily. Individuals may already have an intellectual or developmental disability determination, be in the process of pursuing one, or need support navigating available services and resources.
The INCLUDE team can help determine what supports or referrals may be appropriate based on an individual’s situation.

No. INCLUDE is not an emergency shelter or crisis housing program.
The program focuses on helping individuals access and maintain stable housing through navigation support, coordination of services, and connection to long-term community resources.
Yes. INCLUDE may work alongside other RMHS programs and services depending on an individual’s needs.
The INCLUDE team will help coordinate supports and determine the best approach for each person.
No. INCLUDE does not operate any apartment buildings. Instead, INCLUDE staff can help you find an apartment in the community.

Referrals can be made by anyone including individuals seeking services, family members or caregivers, community partners or providers, or your RMHS case manager.
