National Case Management Week Was Oct. 13-19

RMHS Celebrates the Dedication, Talent and Work of Its Service Coordinators, Intake Coordinators, Care Managers, Transition Specialists and Veteran Support Specialists

Each year during the second week of October, groups across the nation celebrate National Case Management Week to honor and highlight the impact that case managers have on people and communities.

Rocky Mountain Human Services (RMHS) once again joined that celebration this year. RMHS provides personalized case management for children and adults with intellectual and developmental disabilities and their families, military veterans who are experiencing homeless or at risk of homelessness, and people transitioning from inpatient mental health or substance use treatment facilities to the community. Our case management staff surround each child, adult and family with the personalized resources and services they need to achieve their goals.

RMHS began by providing high-quality case management services to children and adults with intellectual and developmental disabilities and have built on that expertise over the years. Our case management staff have many different titles, are located in communities across Colorado and utilize different resources, but the outcome is the same – they work tirelessly to support people to achieve their goals and create their future.
